Healthy Outcomes is offering a Smart Ring giveaway to promote patient wellness and engagement. By participating, you agree to the following terms:
1. Eligibility
Open to legal U.S. residents aged 18 or older. Must be a new patient of Healthy Outcomes and complete the official sign-up process during the giveaway period. Limit one giveaway per household. Void where prohibited.
2. Giveaway Period
Starts June 20, 2025 and ends July 20, 2025, or when 10 eligible signups have been received — whichever comes first.
3. How to Qualify
The first 10 eligible new patients who sign up through the official form on this page will receive one (1) Smart Ring. Signups are time-stamped and monitored to ensure fairness. No purchase necessary.
4. Prize Details
Each eligible participant will receive one (1) Smart Ring (model and size may vary, subject to availability). Approximate retail value (ARV): $150. Prize is non-transferable and not redeemable for cash.
